Purpose
Thank you for taking the time to consider completing this questionnaire!
Your responses help us understand students’ progress, challenges, and growth during tutoring at Brooklyn Math Tutors. These insights allow us to improve our approach and create case studies showcasing real-world impact.
We’re looking for detailed answers—aim for 500+ words (roughly 5-10 minutes of speaking or 15-25 minutes of typing). More is fine if you’re inspired! You don’t need to answer every question; a quick “not applicable” or “nothing comes to mind” is okay for some, but provide enough detail to tell the student’s story.
When to Submit
Please submit this questionnaire only for students whose progress stands out as exceptional. This includes:
- Clear results (e.g., significant grade improvements, test score gains).
- A big turnaround in grades or attitude toward learning.
- An inspiring story that highlights their growth or transformation.
If the student’s story fits these criteria and you believe it’s inspiring or impactful, please let us know via email and complete the questionnaire. You’ll be compensated for your submission as outlined in our email communication, and we may select the student’s story for a case study on our website.
How to Complete
- Access the Student Progress & Impact Questionnaire Google Doc.
- Choose your response method:
- Fill out the Google Doc: Make a copy of the document, complete it, and share the file with team@brooklynmathtutors.com.
- Email answers: Copy the questions, paste them into an email, and send to team@brooklynmathtutors.com.
- Voice memo: Record your thoughts using a tool like iPhone Voice Memos and send the file to team@brooklynmathtutors.com. Voice memos are the easiest way to share detailed insights—speak naturally, and we’ll transcribe it using AI.
- Please submit your completed questionnaire within 2 weeks. Questionnaires submitted significantly after the student finishes working with Brooklyn Math Tutors are less valuable for capturing accurate and timely insights and are not eligible for compensation.
Tips
- Voice memos are recommended for ease and depth. Be conversational, ad-lib, and don’t worry about structure—we’ll organize the details.
- Estimates are fine for logistics (e.g., approximate session dates or number of sessions).
- Focus on the student’s academic progress, mindset changes, and personal growth.
